Ambulatory LPN – Full Time, 8101 Clearvista Drive, Suite 200, Indianapolis, IN.

• Works hand- in- hand with the Physician to achieve quality outcomes for the patient.
• Performs direct and indirect patient care under the supervision of the Registered Nurse and collaborates with RN on the plan of care.
• Assists with activities of daily living, medication administration, and phlebotomy.
• Transcribes orders, documents, and reinforces patient and family education.
• Collaborates with other team members to perform other unit services as directed.

• Graduate of a recognized accredited school of practical nursing, or graduate from a non‑accredited school of practical nursing with three years related professional nursing experience. (Required)
• Previous experience in medical office or health‑care setting. (Preferred)
• Licensed Practical Nurse (LPN) by the Indiana Professional Licensing Agency (IPLA).
