OverviewPNS is a Medicare‑certified home care and home health agency providing total patient & family support for infants, pediatrics, adults, and seniors. We are nurse‑owned and locally led, prioritizing patient care and professional growth.
Service AreaIndianapolis, IN and surrounding 2‑hour radius. Primary service area is Indianapolis and Indianapolis Metro with typical travel distances of 10–60 minutes, occasional travel up to 2 hours. Additional coverage areas available for extra hours if desired.
Job Location OptionsMarion County, Indiana; applicants residing nearby may be considered to meet travel requirements.
SchedulePrimarily weekdays and daytime (M-F). Possible early evenings and rare weekend shifts. Flexible and customizable to accommodate other employment or personal schedules. Most availability required on weekdays day‑time.
Employment TypePRN or part‑time with long‑term employment potential. No on‑call or overnight shifts required. Weekend, overtime, respite & other nursing PRN services offered when available but not mandatory.
Compensation- Hourly wage: $35–$40/hour. Pay weekly on Fridays.
- Round‑trip / door‑to‑door mileage reimbursement provided.
As an RN Case Manager, you will coordinate and provide patient care and ensure optimal outcomes through managed care, patient assessment, treatment planning, and discharge planning. You will handle complex cases, supervise a multidisciplinary team, and communicate effectively with physicians, agency staff, and families.
Responsibilities / Requirements- Perform client assessments; plan and initiate appropriate actions in home care.
- Manage/supervise a team of RN, LPN, Home Health Aide, and Personal Care Aides.
- Document, communicate, and coordinate patient status changes to the care team.
- Promote personal safety and a safe environment for clients and coworkers.
- Travel to surrounding counties.
- Minimum of 1 year of nursing supervisory experience; home‑care experience preferred.
- Experience in case management, OASIS & non‑OASIS assessments, wound care, lab draws, and supervisory roles is a plus.
- Will serve clients of all ages.
- Graduate of an accredited school of nursing.
- Minimum 1 year of experience as a Registered Nurse, with an active RN license in Indiana.
- Minimum 1 year of experience in home health or acute care; 1+ year paid home health experience required; 2+ years preferred.
- Minimum 1 year of experience in case management.
- CPR and first‑aid certifications required (can be obtained prior to hire).
- Strong background in nursing, patient care, confidentiality, attendance, and dependability.
- Current driver’s license and reliable transportation.
- Familiarity with HIPAA regulations and medical terminology.
- Desirable traits: reliable, honest, professional, compassionate, detail‑oriented, accountable, friendly, respectful, and positive.
