Nurse Practitioner; PRN - In-Home Health Assessments

We provide comprehensive health assessments to individuals across the nation, improving health, reducing costs, and empowering people to live their best lives. Our clinicians deliver personalized care by meeting members at their homes or skilled nursing facilities.

• Conduct adult and geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow‑up, and documentation per Matrix guidelines.
• Administer and order point‑of‑care and lab‑based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel.
• Coordinate with the Primary Care Physician on patient education and follow‑up.
• Provide services at the member’s residence, including homes or skilled nursing facilities.

• Flexibility:
  Visits schedule based on your availability from Sunday to Saturday, 8 am–7 pm.
• Competitive Pay:
  Income based on the number of assessments completed. Salary range $1,800–$11,500 per month for 20–100 visits, with per‑assessment rates $90–$115 depending on state.

• Additional Licensure Support:
  Assistance obtaining additional state licensure and credentialing as needed.
• Full‑time Opportunities: PRN providers can transition to full‑time roles with sign‑on bonus.

• Master’s degree or commensurate experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice, including prescription authority or ability to obtain prescriptive authority.
• Board certification (AANP, AACN, or ANCC) in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS, or CPR certification.
• One year of NP experience preferred; new graduates encouraged.
• Valid state driver’s license and automobile insurance coverage for the state of residence.
• Coverage requirements:
  Bodily Injury $100,000 per person / $300,000 per incident;
  Property Damage $50,000;
  Collision and comprehensive deductibles $1,000 or less.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.
