Our clinicians deliver personalized care by meeting members at their homes or skilled nursing facilities.
Responsibilities- Conduct adult and geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow‑up, and documentation per Matrix guidelines.
- Administer and order point‑of‑care and lab‑based testing as needed.
- Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel.
- Coordinate with the Primary Care Physician on patient education and follow‑up.
- Provide services at the member’s residence, including homes or skilled nursing facilities.
- Flexibility:
Visits schedule based on your availability from Sunday to Saturday, 8 am–7 pm. - Competitive Pay:
Income based on the number of assessments completed. Salary range $1,800–$11,500 per month for 20–100 visits, with per‑assessment rates $90–$115 depending on state. - Benefits:

Comprehensive onboarding, preceptor assignment, 1:1 feedback, and free continuing‑education access. - Additional Licensure Support:
Assistance obtaining additional state licensure and credentialing as needed. - Full‑time Opportunities: PRN providers can transition to full‑time roles with sign‑on bonus.
- Master’s degree or commensurate experience and satisfactory completion of NP licensure.
- Current NP licensure in state of practice, including prescription authority or ability to obtain prescriptive authority.
- Board certification (AANP, AACN, or ANCC) in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
- Current BLS, ACLS, or CPR certification.
- One year of NP experience preferred; new graduates encouraged.
- Valid state driver’s license and automobile insurance coverage for the state of residence.
- Coverage requirements:
Bodily Injury $100,000 per person / $300,000 per incident;
Property Damage $50,000;
Collision and comprehensive deductibles $1,000 or less. - Strong computer skills and familiarity with employee health/medical record software.
