Silver Memories Health Care is seeking a dedicated and detail-oriented Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as a Registered Nurse Assessment Coordinator (RNAC / MDS Coordinator). This part‑time position is ideal for an experienced MDS professional looking for flexibility while making a meaningful impact on resident care and regulatory compliance.

For candidates seeking a full-time opportunity, this role may be expanded to include Infection Prevention (IP) and/or Wound Nurse responsibilities based on experience and qualifications.

Your Role & Responsibilities

As the RNAC / MDS Coordinator, you will:

  • Conduct resident assessments according to established schedules and in response to changes in condition, hospital readmissions, and other significant clinical events.

  • Complete MDS assessments, Care Area Assessments (CAAs), care plans, and supporting documentation in compliance with federal and state regulations.

  • Develop, update, and monitor individualized resident care plans in collaboration with the interdisciplinary team.

  • Coordinate with Nursing, Therapy, Social Services, Dietary, and Activities to ensure timely completion of assessments and care planning.

  • Accurately observe, assess, and communicate changes in resident condition.

  • Prepare and maintain the MDS schedule while ensuring timely transmission of MDS data.

  • Maintain accurate documentation and support facility quality measures and regulatory compliance.

For candidates interested in a full‑time opportunity, additional responsibilities may include:

  • Serving as the facility's Infection Preventionist (IP) by assisting with infection surveillance, prevention initiatives, staff education, and regulatory compliance.

  • Providing Wound Nurse services, including wound assessments, treatment coordination, documentation, physician communication, and monitoring resident progress.

What We’re Looking For
  • Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Indiana.

  • Minimum of one (1) year of prior MDS experience in a skilled nursing or long‑term care setting.

  • Working knowledge of MDS processes, RAI guidelines, CAAs, care planning, and Medicare/Medicaid reimbursement.

  • Experience with Infection Prevention and/or wound care is preferred for candidates interested in expanding to a full‑time role but is not required.

  • Strong organizational, communication, and time management skills.

  • Ability to work independently while collaborating effectively with the interdisciplinary team.
