Deliver vital healthcare services and make a meaningful impact as an experienced Nurse Practitioner performing home health risk assessments for Medicare Advantage plan members. This per diem opportunity provides flexibility and autonomy, allowing you to tailor your schedule and contribute to better healthcare outcomes within the community.

• Flexible shifts available Monday through Sunday
• Self-scheduling to fit your lifestyle
• Minimum weekly or bi-weekly hourly commitment

• Perform comprehensive annual health and post-discharge assessments in patient homes across Howard County, including Kokomo, Greentown, and Russiaville
• Evaluate medical histories, current health conditions, and medication regimens
• Educate members about their medical conditions and medications to promote self-management and understanding
• Identify gaps in care, providing actionable recommendations to address potential risks
• No requirement to develop treatment plans or prescribe medications

• Active Nurse Practitioner license in Indiana
• Board Certification via AANP or ANCC
• Current CPR certification (BLS required)
• MSN degree from an accredited nursing program
• At least six months experience as a Nurse Practitioner preferred
