Key Responsibilities- Collaborate with patients and multidisciplinary teams to develop personalized treatment plans
- Conduct assessments, diagnoses, and treatments
- Create weekly rounding schedules for facilities
- Maintain accurate medical records
- Utilize current guidelines to provide optimal care
- Certified Nurse Practitioner certification
- Preferably 2+ years in Skilled Nursing facilities
- Strong communication, organizational, and time management skills
- Ability to travel within a 12-20 mile radius
- Competitive pay starting at $50/hour
