The Licensed Practical Nurse, working in collaboration with an RN, performs independent advanced nursing functions, including assessment, planning, implementation, and evaluation of assigned patient care. The role supports patient care within the Opioid Treatment Program and operates within established guidelines and after demonstrating clinical competency.
Essential Functions and Responsibilities- Advocates, cares, teaches and serves a vulnerable population, aligning with Eskenazi values.
- Assesses, plans, implements, and evaluates patient care in collaboration with an RN.
- Maintains responsibility for all assigned patient care and adheres to standards of practice with RN support.
- Assists in a health‑care team approach, performs client examinations, treatments, and specialized procedures per guidelines.
- Administers prescribed treatments and/or medications, after demonstrating clinical competency and following policy.
- Performs intake processes, including medical history gathering, lab work scheduling, withdrawal symptom documentation, and client orientation.
- Supports team meetings, providing insight useful to counselors and for client‑care management.
- Communicates effectively; standing, walking, lifting, and stooping as required.
- Adjusts activities and behaviors to ensure appropriate care for age‑specific patients.
- Evaluates clients to determine if a higher level of medical care is required.
- Graduate from an accredited Licensed Practical Nurse training program or equivalent, with completion of pharmacology coursework.
- Minimum six months of clinical nursing experience with patients of all age groups to qualify for LPN II.
- Current Indiana License as a Licensed Practical Nurse (or eligible for license) and current CPR certification.
- Proficient in patient care delivery and use of unit‑appropriate equipment and instruments.
- Strong interpersonal communication skills and accurate record‑keeping.
- Ability to follow oral and written instructions and collaborate with diverse individuals.
- Capability to function effectively in stressful or crisis situations.
- Flexibility to changing work priorities and responsiveness to patient needs.
- Knowledge of age‑appropriate care and growth/development principles across the lifespan.
- Experience and training in safely managing therapeutic services for opioid treatment patients.

Sandra Eskenazi Mental Health Center is dedicated to serving individuals with serious mental illness, chronic addiction, and emotionally disturbed children and families.
