Reports To: Clinical Supervisor
Altra Home Health is seeking a compassionate and skilled Nurse to join our team. In this rewarding role, you’ll provide personalized care to
pediatric patients in their homes — many of whom have complex medical conditions
, including gastrostomy tubes (G-tubes), cerebral palsy, chronic diseases
, and occasionally require tracheostomy or ventilator support
. You’ll play a vital role in helping them live safely and comfortably in their home, where they thrive the most.
- Perform thorough head-to-toe assessments and follow individualized plans of care
- Provide care which may include G-tube feedings, medication administration, wound care, trach care and ventilator support.
- Care for patients with professionalism and empathy.
- Educate patients and caregivers on disease management, emergency procedures, and therapeutic techniques
- Collaborate closely with physicians, therapists, and other team members to optimize care
- Document all care activities accurately and promptly in compliance with agency and state requirements
- Maintain rigorous safety, infection control, and privacy standards
- Shift durations range from 4 to 12 hours
, depending on patient needs and nurse availability - Flexible scheduling options available for daytime, nighttime, evening, and weekend shifts
- Opportunities for consistent long‑term cases or variable schedules
- Active Nursing license in Indiana or an active compact Nursing license
- At least [1–2] years of clinical nursing experience (home health and pediatric experience strongly preferred)
- Experience with G-tube, trach, or vent care preferred (but training will be offered)
- Current CPR certification
- Strong critical thinking and independent decision‑making abilities
- Excellent interpersonal and communication skills
- Reliable transportation and valid driver’s license
