DaVita SNF Dialysis RN – responsible for patient care and dialysis treatment in a skilled nursing facility setting.

In this role, you will be responsible for the complete ongoing care of each patient to keep dialysis treatments on track. You will troubleshoot and make sound decisions about dialysis treatment, occasionally partnering with SNF staff to coordinate care.

Patient

Education:

As a RN, you will serve as an educator, providing meaningful education to patients and families within the SNF setting.

Direct Patient Care:
You will independently demonstrate best practice techniques and deliver optimal dialysis care, supervising Dialysis Technicians who support patient treatments.

Complexity and Multitasking:
Dialysis care is complex; meeting clinical outcomes and handling multiple patients simultaneously is part of the job.

Autonomy & Critical Thinking:
Work independently, assess, diagnose, and implement comprehensive care plans, continuously applying core nursing skills to protect patient outcomes.

• Current Registered Nurse (RN) license in the state of practice
• Minimum of 12 months of RN experience (Dialysis, hospital or critical care highly preferred)
• Associate’s Degree in Nursing (ADN) from an accredited program; a Bachelor of Science in Nursing (BSN) or a three‑year accredited diploma may substitute
• Basic computer skills and proficiency in Microsoft Word and Outlook

• Experience with Chronic Hemodialysis, Acute Dialysis and/or Peritoneal Dialysis (PD)
• Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN)
• Background in training, teaching, or healthcare education
