Licensed Practical Nurse

Trilogy Health Services is seeking a Licensed Practical Nurse to join our Healthcare & Medical Services team. In this role, you'll provide direct nursing care to seniors, administer medications, perform treatments, and monitor resident conditions in collaboration with RNs and physicians. You'll document care accurately, support care plan development, and educate residents and families. Responsibilities

• Provide direct bedside nursing care to senior residents.
• Administer medications and treatments per physician orders and facility policies.
• Monitor, document, and report changes in resident conditions.
• Participate in developing, implementing, and updating individualized care plans.
• Collaborate with RNs, physicians, therapists, and other care team members.
• Maintain accurate, timely electronic medical records and documentation.
• Perform basic wound care, catheter care, and other LPN-level procedures.
• Educate residents and families on care plans, medications, and health needs.
• Maintain infection control and safety standards at all times.
• Support a compassionate, resident-centered environment aligned with Trilogy's values.
