Job Title: Home Health Registered Nurse (RN)
Assignment Length: 13 Weeks
Schedule: Monday – Friday (No On-Call)
We are seeking an experienced Home Health Registered Nurse (RN) for a 13-week assignment in the Fairfax, VA area. This position offers a consistent Monday–Friday schedule with no on-call requirements, making it ideal for nurses seeking work-life balance while delivering high-quality patient care in a home setting.
ResponsibilitiesProvide skilled nursing care to patients in their homes
Perform patient assessments and develop individualized care plans
Complete accurate and timely documentation using HCHB (Homecare Homebase)
Conduct OASIS assessments and ensure compliance with regulatory standards
Educate patients and families on disease management and care plans
Collaborate with physicians, therapists, and interdisciplinary team members
Monitor patient progress and adjust care plans as needed
RequirementsActive Registered Nurse (RN) license in IN
Minimum 1 year of home health experience within the last 3 years
HCHB (Homecare Homebase) experience required
OASIS experience required
Current BLS certification (AHA preferred)
Valid driver’s license and reliable transportation
