We are seeking a Licensed Practical Nurse (LPN) to provide in-home patient care.
Responsibilities include utilizing the nursing process to plan, implement, and evaluate patient care, initiating and revising care plans as needed, administering medications and treatments, responding to changes in patient conditions, and coordinating education and discharge planning.
Qualifications required are a current LPN/LVN license, TB or Chest X-Ray, BLS certification, and preferably one year of nursing experience.
Skills include strong clinical judgment, effective communication, and the ability to respond quickly to patient needs.
Key duties involve medication administration, patient monitoring, reporting changes to physicians, and ensuring quality care delivery.
