Location: Indianapolis
Community Health Network is seeking an Ambulatory Triage Registered Nurse to join the Diabetes & Endocrinology Care team in Indianapolis. The role includes phone triage, patient education, care coordination, referrals, and occasional direct patient care including nurse visits and injections.
The ideal candidate has RN licensure in Indiana, 1+ year nursing experience, and preferably diabetes/endocrinology or medical office exposure.
