Monday-Friday, 8:00 AM-4:30 PM
Weekly Hour Guarantee: 40 Hours
Productivity Expectation: 25-30 Points Per Week

We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to join our team serving patients throughout Marion, Hamilton, and Johnson Counties. This role is ideal for a clinician who thrives in an autonomous environment, delivers exceptional patient care, and is experienced in home health documentation and OASIS assessments.
The RN will be responsible for conducting comprehensive patient assessments, coordinating care plans, and providing skilled nursing services to patients in their homes.
Key Responsibilities- Complete OASIS Start of Care (SOC) assessments and admissions.
- Perform recertification visits and evaluations in accordance with agency and regulatory requirements.
- Conduct patient discharge visits and complete all necessary documentation.
- Medication management and medication set-up.
- Wound assessment, treatment, and ongoing wound care management.
- Monitor patient progress and identify changes in condition.
- Collaborate with physicians, interdisciplinary team members, patients, and caregivers to ensure continuity of care.
- Educate patients and families on disease management, medications, safety, and treatment plans.
- Maintain accurate, timely, and compliant clinical documentation within the electronic medical record system.
- Ensure adherence to all agency policies, state regulations, and home health standards.
- Active and unrestricted Registered Nurse (RN) license.
- Previous Home Health Experience Required.
- Experience completing OASIS assessments, including Start of Care, Recertification, and Discharge visits.
- Proficiency with Homecare Homebase (HCHB) required.
- Strong clinical assessment and critical thinking skills.
- Reliable transportation and ability to travel throughout the assigned coverage area.
