We are looking for a Full Time/Weekend Night Licensed Practical Nurse to join our Hospice House team!
The LPN will provide nursing care to the terminally ill Hospice patient as needed. This is performed in accordance with physician orders and IDG plan of care under the direction and supervision of the Registered Nurse. To provide assistance and understanding to the family in time of bereavement. To work as a member of the Hospice team in providing Hospice care.
Essential
Functions/Responsibilities:
Regularly assesses and reassesses the nursing needs of hospice patients and communicates changes in condition to the physician and appropriate members of the Interdisciplinary Group (IDG) or other hospice providers.Provides skilled hospice nursing services, treatments, symptom management, and preventive procedures in accordance with the established plan of care and physician orders.Initiates and performs nursing interventions and procedures appropriate to the patient’s needs, comfort, safety, and goals of care.Implements and evaluates, and updates the patient/family plan of care in collaboration with the IDG to address identified needs and ensure continuity of care.Observes and evaluates the patient’s physical, psychosocial, and emotional status and promptly reports significant or unexpected changes to the physician, supervisor, and appropriate IDG members or other hospice providers.Provides education, instruction, counseling, and support to patients, families, and caregivers regarding the patient’s condition, medications, symptom management, safety, and provision of care.Provides dietary education and counseling within the scope of nursing practice and coordinates additional nutritional support as appropriate.Supervises, educates, and supports nursing personnel and other assigned clinical staff in accordance with agency policy and applicable regulatory requirements.Participates in the orientation, education, teaching, and training of staff as requested.Collaborates effectively with the IDG and other healthcare professionals to coordinate patient-centered hospice care and services.Attends team conferences, in-services, and other required meetings as scheduled.Maintains accurate, timely, and complete clinical documentation and records in accordance with agency policies, professional standards, and applicable regulatory requirements.Adheres to all agency policies and procedures and maintains patient confidentiality in accordance with HIPAA and other applicable privacy requirements.Complies with agency infection prevention and control policies, procedures, and protocols.Immediately communicates with the appropriate supervisor when unable to meet a patient’s identified need, or safely perform a required procedure.Maintains the clinical knowledge, competencies, licensure, and skills necessary to perform assigned responsibilities safely and effectively.Organises and prioritises assigned responsibilities, utilizing effective time-management skills to complete patient care duties, and documentation.Performs other duties and responsibilities as assigned by the Administrator and/or Inpatient Director.Qualified applicants will possess the following:
- Graduate from an accredited school of nursing
