Role: Licensed Practical Nurse (LPN) in a long-term care and post-acute community.
Responsibilities- Provide resident-centered, compassionate nursing care.
- Administer medications and treatments accurately.
- Monitor residents’ conditions and document in the Electronic Health Record (EHR).
- Collaborate with an interdisciplinary team to ensure optimal care outcomes.
- Mentor Certified Nursing Assistants (CNAs) and support team development.
- Build meaningful relationships with residents and their families.
- Support dignity, safety, and quality of life for residents.
- Licensed Practical Nurse (LPN) licensure.
- Strong compassionate and communication skills.
- Ability to work collaboratively in a team-oriented, faith-based environment.
- Commitment to person-centered care and continuous improvement.
