PRN RN Case Manager – Home Health

This leadership position is a PRN RNCM service with long‑term employment potential. The role offers fast growth to Part‑Time, Full‑Time, Contractor or Hybrid RNCM positions as the agency expands.

Travel to clients throughout Indiana is required. The RNCM reports directly to the Indiana Regional Program Administrator and the Director of Clinical & Administrative Operations.

A defined career path is available to advance to Assistant Program Administrator, Program Administrator, Clinical Administrator, Director of Nursing, Business Administrator, QA/Compliance leadership, and other roles.

• Case management, care starts of care (SOC), admissions, recertifications, transitional care, resumption of care (ROC), clinical reviews, and other assessments.
• New hire orientations, in‑service and supervisory reviews, and providing input to influence training, education, and care practices.
• Occasional skilled nursing visits, weekly meetings with interdisciplinary teams, and communication with physicians, agency staff, service administrators, and client families.
• Travel up to 2 hours to cover the service area and occasional longer trips.

• Graduate of an accredited School of Nursing.
• Minimum of 1 year as a registered nurse with an active RN license in Indiana.
• Minimum 1 year of paid home health or in‑home care experience (2+ years preferred).
• Minimum 1 year of case management experience.
• Experience with OASIS and non‑OASIS assessments, wound care, lab draws, and PICC/PORT/IV care.
• CPR and First Aid certifications.
• Strong communication, documentation, and confidentiality skills.
• Reliable transportation and current driver’s license.
• Desirable traits: compassionate, detail oriented, accountable, friendly, respectful, positive.

• All capacities available – Part‑Time, Full‑Time, PRN, Contractor, or Hybrid.
• Primarily weekdays daytime (Mon‑Fri, 8 am‑5 pm). Early evening and rare weekend shifts may be required.
• No on‑call, overnight, or overtime shifts are required. Occasional additional visits may be offered.

• Service area:
  Indianapolis, IN and surrounding 2‑hour radius. Typical travel distances 10–60 minutes; occasional travel up to 2 hours.

• Hourly wage: $36–$40 per hour.
• Full‑time RNCMs may be eligible for a salary paid twice monthly.
