Logan Health Hospice is looking for an independent, self-directed RN with strong clinical nursing skills to be part of their compassionate Interdisciplinary Team! Working for hospice puts you back where nursing started: at the bedside.
You'll build real relationships with patients and families, guiding them through symptom management, education, and comfort care at the end of life's journey.
- You’ll manage your own patient caseload and travel throughout the Flathead Valley visiting patients where they call home.
- Be the primary nursing contact for patients, coordinating their care from admission through end of life Conduct in-home visits for assessment, symptom management, and comfort-focused interventions
- Educate patients, families, and caregivers on what to expect and how to provide care and symptom management
- Collaborate with our interdisciplinary team — physician, social workers, chaplains, CNAs, LPNs, and volunteers to create a customized patient plan of care
- Attend regularly scheduled Interdisciplinary Team (IDT) meetings
- Provide emotional support and steady presence for families navigating anticipatory grief
- Participate in on-call shift rotation and perform ad-hoc patient visits as needed
- Document visits and coordinate medications, equipment, and supplies to keep care seamless
- Must be comfortable driving in winter weather and have a reliable vehicle for transportation.
- Mileage reimbursement $100 per month vehicle stipend

Qualifications:- Current Montana RN license or a multi-state compact license with authorization to practice nursing within the state of Montana required.
- BSN degree preferred.
- Minimum of two (2) years’ experience in a similar position within a medical setting (e.g., physician office, clinic, or hospital) preferred.
- Experience in assigned area(s) preferred.
- Current Driver’s License required.
- Driving record for the previous thirty-six (36) months must not reflect any major driving offenses (i.e; DUI, convicted of a felony involving a motor vehicle, suspended license, three (3) or more speeding tickets or other moving violations, etc.).
- In addition, personal vehicles used for LH business:
State law minimum liability coverage limits required. - Possess knowledge of professional nursing theory, medical terminology, and principles of professional nursing practice to administer and evaluate patient care.
- RQI Provider (BLS) required within 7 days of hire/transfer or before independent patient care, whichever occurs first.
