Case Manager

Our client, a dedicated organization committed to empowering individuals and families, is seeking a Case Manager/Success Coach to join their team. As a Success Coach, you will be part of the Coaching for Success program supporting community members in their journey toward self-sufficiency. The ideal candidate will have strong communication skills, empathy, and adaptability, which will align successfully in the organization.

Job Title: Case Manager /Success Coach

Location: Columbus, IN Northside

Pay Rate:$19.50/hour

Shift:Monday - Friday, day shift

What's the Job?

• Support individuals and families in setting and achieving SMART goals related to income, education, and housing.
• Conduct strengths and needs assessments, and follow-up assessments every 90 days.
• Coach participants in budgeting, time management, soft skills, and building self-confidence.
• Support participants during crisis situations and coordinate with partner agencies as needed.
• Maintain a participant-centered, trauma-informed, and culturally responsive approach at all times.

What's Needed?

• High School Diploma or equivalent.
• Valid driver’s license, reliable personal vehicle, and proof of insurance.
• Ability to travel throughout the service region; mileage reimbursed at the IRS maximum rate.
• Proficiency with Microsoft Office (Word, Excel, Outlook, PowerPoint, Publisher).
• Strong written and verbal communication skills.
