Position Title: Family Resource Specialist
Employment Type: Full-Time
Location: Indianapolis, IN
The Family Resource Specialist serves as a key support professional for individuals and families by connecting them with community resources, providing case management services, and promoting family stability and well-being. This role works collaboratively with families, community partners, schools, healthcare providers, and social service agencies to assess needs, develop service plans, and ensure access to available supports.
Key Responsibilities- Conduct comprehensive assessments to identify family strengths, needs, and goals.
- Develop individualized service plans and monitor progress toward established objectives.
- Connect families with community resources, including housing, food assistance, healthcare, childcare, employment, transportation, and educational services.
- Provide advocacy and support to help families navigate social service systems.
- Maintain accurate and timely documentation, case notes, referrals, and reports.
- Facilitate workshops, support groups, and educational programs for families.
- Build and maintain partnerships with community organizations and service providers.
- Conduct outreach activities to engage underserved populations.
- Monitor and evaluate service effectiveness and client outcomes.
- Participate in team meetings, training opportunities, and program improvement initiatives.
- Ensure compliance with organizational policies, confidentiality standards, and applicable regulations.
- Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Education, or a related field.
- Minimum of 1–2 years of experience working with children, families, or vulnerable populations.
- Knowledge of community resources and social service systems.
- Strong interpersonal, communication, and problem-solving skills.
- Ability to work effectively with diverse populations and cultural backgrounds.
- Proficiency with Microsoft Office and case management software.
- Valid driver's license and reliable transportation (if required).
- Bilingual or multilingual abilities.
- Experience in case management, family support services, or community outreach.
- Knowledge of trauma-informed care and family-centered practices.
- Certification in social services, family support, or related fields.
