Silver Memories Healthcare is seeking a dedicated Social Services Director to join our skilled nursing facility in Versailles, IN.
The Social Services/Activity Director serves as a liaison and advocate for residents’ social interests and is responsible for planning, organizing, implementing, evaluating, and directing the overall operations of the Social Services Department.
Position Details- Full-Time
- Develop comprehensive social assessments and care plans identifying residents’ medically related social and emotional needs with realistic goals and interventions
- Actively participate in pre-admission and admission processes
- Provide compassionate support to residents and families during adjustment, crisis, or special circumstances
- Maintain accurate documentation of residents’ behavioral and psychosocial status
- Provide individual and group support services for residents and families
- Establish and grow a volunteer program to meet social service needs
- Orient residents and families to the facility and assist with transition and adjustment
- Plan and schedule engaging activities such as movies, parties, games, crafts, projects, and field trips, with activity calendars prepared at least one month in advance
- Assess and monitor residents’ spiritual, social, recreational, and emotional needs through routine visits and discussions with residents and their families
- Encourage residents to improve physical, cognitive, and social skills through participation in activities
- Collaborate with nursing supervisors to ensure activities align with physicians’ orders and care plans
- Assist with resident shopping requests and support facility programs as needed
- Coordinate and facilitate monthly Resident Council meetings
- Document resident participation and activity updates in PCC (Point Click Care )
- Work with the interdisciplinary team to develop meaningful, resident-centered programs
- Strong knowledge of social service practices, procedures, laws, and regulations in long-term care
- Ability to work independently with strong interpersonal and communication skills
