Location: Indianapolis
Horizon House in Indianapolis conducts community-based outreach to unsheltered individuals throughout Indianapolis and Marion County. The Street Outreach Professional engages clients, assesses needs, and connects them to housing-focused and supportive services within a harm-reduction framework.
This grant-funded, at-will role supports collaboration with partners and requires documentation in HMIS. Schedule is Saturday through Wednesday, with Thursdays/Fridays off; reports to the Street Outreach
