A community-based provider who conducts social services in the home to families referred through the Indiana Department of Child Services (DCS) to Cornerstone Support Services. The goal of the services is to maintain children in their own home with the introduction of appropriate services to the family. Hiring in Region 1 (Lake County), Region 10 (Marion County), Region 11 (Hamilton, Tipton, Madison, Hancock Counties), Region 16 (Gibson, Knox, Pike, Posey, Vanderburgh, Warrick Counties), Region 17 (Crawford, Daviess, Dubois, Martin, Orange, Perry, Spencer Counties), Region 18 (Clark, Floyd, Harrison, Scott and Washington Counties).v

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • 1. Conducts a multi-level family assessment, develops a service/treatment plan and provides an array of needed services focused on preserving the family and avoiding removal of the children from their home, provided it is safe for the children to remain with their identified caregivers.

  • 6. Conducts timely performance reviews.
  • 7. Conduct staff counseling and corrective action including work improvement plans and follow-up.
  • 8. Demonstrates the ability to teach and model skills from evidenced based practices. Training to facilitate the parent’s development of special skills or increase knowledge in the areas of parenting, home management, behavior management, and child development.
  • 9. Perform at least 1 home visit per week (or more if requested by the Indiana Department of Child Services) monitoring and addressing any safety concerns for the children. Interventions must be strength- based and family driven with the family actively participating in identifying the focus of services.
  • 10. Conducts services that are sensitive with respect to each family’s culture, ethnic origin, race, language, gender, age, religion, socioeconomic status, sexual orientation, mental and/or physical challenges. Provides services in a natural environment convenient to the family.
  • 11. Provides advocacy on behalf of families with schools and other agencies available in the community. Assists consumers in the pursuit of needed resources or services. Arranges for any follow-up services needed following the end of services.
  • 12. Demonstrates the ability to teach communication and negotiation skills, link and refer the families with needed community resources and supports.
  • 13. Works a 40-hour full-time schedule being available to families 24 hours per day, seven days per week, 365+ days per year. Evening and weekend hours will be required to accommodate the work schedule of each caregiver and school schedules of the children in the family when scheduling appointments.
  • 14. Work with the family case manager and the Indiana Department of Child Services (DCS) in the creation of treatment and safety plans.
  • 15. Must actively engage in the Indiana Department of Child Services (DCS) Practice Model and attend scheduled Child and Family Team Meetings (CFTMs) whenever requested. Willingness to support the team process and participate in continuous development of family goals.
  • 16. Utilize the outlook calendar to submit a weekly schedule to the supervisor which outlines scheduled appointments for the week. The schedule must include the family name and case number, current address and telephone number. The Family Preservation Specialist must notify the Supervisor of any alterations to the schedule (cancellations, new appointments, missed appointments etc.).
