Location: Indianapolis
Aspire Indiana Health in Indianapolis is seeking a Contract Case Manager to support shelter residents by coordinating health, housing and community resources. You will work in person at the shelter, build relationships with service providers, and develop individualized housing stability plans.
This temporary contracted role requires strong documentation, crisis intervention, and close collaboration with Aspire staff.
