Porter-Starke Services in Knox, Indiana, is seeking a Housing Navigator to provide full-time, community-based case management and outreach for adults with serious mental illness and/or substance use disorders. The role supports the Rural Set-Aside Grant and MRO services to help clients achieve housing stability and community integration.
The Housing Navigator will work with partners to locate, coordinate, and monitor services, assist with treatment planning, and help clients access entitlements
