Overview
Join our dynamic healthcare team as a Home Health Clinical Manager
- Registered Nurse, where you will lead and coordinate comprehensive home health services with enthusiasm and expertise. This pivotal role combines clinical excellence with administrative leadership, ensuring patients receive top-quality care in the comfort of their homes. You will oversee daily operations, manage clinical staff, and ensure compliance with healthcare standards, all while fostering a compassionate environment that puts patient needs first.

Responsibilities
  • Lead the planning, implementation, and evaluation of home health services to ensure high-quality patient care aligned with regulatory standards.
  • Manage clinical staff including nurses, therapists, and support personnel, providing mentorship and performance feedback to promote professional growth.
  • Oversee patient assessments, care planning, and documentation to ensure accurate medical records using EMR (Electronic Medical Records) systems such as Kantime, Epic or Cerner.
  • Coordinate interdisciplinary team efforts for complex cases involving long-term care, hospice & palliative medicine, or acute pain management.
  • Ensure compliance with HIPAA regulations and maintain confidentiality of all patient information while adhering to OSHA standards and infection control protocols.
  • Facilitate staff training on aseptic techniques, IV infusion procedures, wound suturing, catheter care, and other clinical skills necessary for home-based care.
  • Conduct quality assurance audits in accordance with NCQA standards and monitor patient outcomes to optimize service delivery.
Qualifications
  • Valid Registered Nurse (RN) license with current state certification.
  • Proven leadership experience in home health or related settings such as hospitals, nursing homes, or assisted living facilities.
  • Extensive knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation methods including ICD coding (ICD-9/ICD-10).
  • Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinical

    Works for documentation and patient management.
  • Experience in diverse clinical areas including critical care (ICU), emergency medicine, cardiology (cardiac catheterization), nephrology (dialysis), geriatrics (memory & dementia care), behavioral health, pediatrics (neonatal ventilator), and post-acute care environments.
  • Strong organizational skills with the ability to manage multiple priorities such as discharge planning and case management efficiently.
  • Experience in the development and execution of the IDOH, CMS, and CHAP credential process
  • Excellent communication skills to collaborate effectively with patients, families, healthcare providers, and community resources.
