Responsibilities

Lead the planning, implementation, and evaluation of home health services to ensure high-quality patient care aligned with regulatory standards.
Manage clinical staff including nurses, therapists, and support personnel, providing mentorship and performance feedback to promote professional growth.
Oversee patient assessments, care planning, and documentation to ensure accurate medical records using EMR (Electronic Medical Records) systems such as Kantime, Epic or Cerner.
Coordinate interdisciplinary team efforts for complex cases involving long-term care, hospice & palliative medicine, or acute pain management.
Ensure compliance with HIPAA regulations and maintain confidentiality of all patient information while adhering to OSHA standards and infection control protocols.
Facilitate staff training on aseptic techniques, IV infusion procedures, wound suturing, catheter care, and other clinical skills necessary for home-based care.
Conduct quality assurance audits in accordance with NCQA standards and monitor patient outcomes to optimize service delivery.

Qualifications

Valid Registered Nurse (RN) license with current state certification.
Proven leadership experience in home health or related settings such as hospitals, nursing homes, or assisted living facilities.
Extensive knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation methods including ICD coding (ICD-9/ICD-10).
Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinical

Works for documentation and patient management.
Experience in diverse clinical areas including critical care (ICU), emergency medicine, cardiology (cardiac catheterization), nephrology (dialysis), geriatrics (memory & dementia care), behavioral health, pediatrics (neonatal ventilator), and post-acute care environments.
Strong organizational skills with the ability to manage multiple priorities such as discharge planning and case management efficiently.
Experience in the development and execution of the IDOH, CMS, and CHAP credential process
