In This Role You Will:

The Registered Nurse Case Manager
- Home Health

* Perform comprehensive patient assessments and complete all required OASIS.

* Develop, implement, and manage individualized Plans of Care in collaboration with physicians and the interdisciplinary team.

* Provide skilled nursing care and serve as the primary case manager, coordinating services, monitoring patient progress, and communicating changes promptly.

* Educate patients and caregivers on health conditions, safety, and self-care.

* Ensure accurate, timely documentation and compliance with Medicare, state, and agency requirements.

* Participate in case conferences and quality initiatives.

* Provide on-call coverage, including evenings and weekends, as assigned.

Shift Details:

This is a full-time, Monday through Friday position with daytime hours. While schedules offer flexibility to accommodate patient needs, RN Case Managers participate in a rotating on-call schedule, which includes evenings and weekends. Travel within our service area is required.

Who You Are:

* Current Registered Nurse (RN) license in the state of Indiana.

* Minimum of 1-2 years of nursing experience (home health or acute care preferred).

* Knowledge of OASIS documentation and Medicare home health regulations preferred.

* Strong clinical assessment, critical thinking, and problem-solving skills.

* Excellent organizational and communication skills.

* Ability to work independently and manage a caseload effectively.

* Valid driver's license, reliable transportation, and proof of auto insurance.
