In This Role You Will:

The Registered Nurse
- Home Health

* Review referrals and coordinate client eligibility and care with families and healthcare team

* Conduct initial and ongoing assessments, including OASIS data collection

* Develop, follow, and update individualized Plans of Care

* Provide skilled interventions and report changes to update care plans

* Maintain accurate and timely documentation

* Supervise and support RNs, LPNs, and Home Health Aides, including training and productivity monitoring

* Ensure patient and staff safety and follow infection control practices

* Participate in team meetings, care conferences, and quality improvement initiatives

* Maintain confidentiality and comply with regulatory and Agency standards

Shift Details:

This is a part-time RN role offering a flexible schedule with shifts arranged throughout the week. The position involves providing care to patients in their homes, traveling within the Frankfort, IN area. The RN will participate in an on-call rotation to support patients as needed.

Who You Are:

* Current Registered Nurse (RN) license in the state of Indiana.

* Minimum of 1-2 years of nursing experience (home health preferred).

* Knowledge of OASIS documentation and Medicare home health regulations preferred.

* Strong clinical assessment, critical thinking, and problem-solving skills.

* Excellent organizational and communication skills.
