Job Description:

Looking for a nursing role that keeps you involved in patient care without working bedside shifts?

As a Pre-Admission Coordinator, you'll play a key role behind the scenes by reviewing referrals, coordinating admissions, and helping patients transition smoothly into home health services. This is a great opportunity for an LPN who enjoys organization, communication, and being part of a collaborative clinical team.

What You'll Do

Review patient records prior to admission

Ensure clinical documentation is complete and accurate

Coordinate with referral sources, facilities, and clinical teams

Track pending admissions and discharge timelines

Support smooth transitions into home health services

Communicate with patients, families, and healthcare partners

Utilize systems such as Homecare Homebase and Workday

What You Need

Active LPN/LVN license

1+ year of healthcare experience

Home Health or Hospice experience preferred

Strong communication and organizational skills

Ability to multitask and problem solve in a fast-paced environment

Basic Microsoft Office skills and ability to learn EMR systems

Bilingual English/Spanish preferred

Valid driver's license and reliable transportation
