We are seeking an experienced Registered Nurse (RN) to support and oversee high-quality in-home patient care. This role is responsible for clinical oversight, quality assurance, patient and caregiver satisfaction, and effective coordination of care across multiple locations.

- Utilize effective problem-solving and clinical decision-making skills to identify issues, determine root causes, develop solutions, and evaluate outcomes
- Conduct client visits and participate in quality improvement initiatives to ensure service satisfaction
- Investigate and resolve assigned patient and staff complaints in a timely and professional manner
- Ensure all clinical documentation is accurate, complete, timely, and compliant with company policies, procedures, and regulatory requirements
- Assess patient status and interpret clinical data to identify age-specific and diagnosis-related care needs
- Coordinate and communicate effectively with patients, families, caregivers, and interdisciplinary care teams
- Provide oversight and guidance to clinical staff as needed to support quality patient outcomes

- Geriatric
- Active and unrestricted Registered Nurse (RN) license in the state of Indiana
- Minimum of two (2) years of home health nursing experience (required)
- Valid driver's license and reliable personal vehicle
- Demonstrated clinical knowledge and skills necessary to provide and coordinate patient care
- Ability to assess patient data and identify appropriate care needs based on age and diagnosis
- Strong communication, organization, and problem-solving skills
- Proficient computer skills and experience with electronic documentation systems
