Registered Nurse RN Home Health | 1:1 Patient Care

What you will do

* Manage and coordinate your own patient caseload

* Complete patient assessments and reassessments

* Develop and update individualized care plans

* Perform start of care and follow up visits

* Communicate changes in patient condition with physicians and the care team

* Collaborate with an interdisciplinary team to improve patient outcomes

What you need

* Active RN license

* 1+ year of clinical nursing experience

* Ability to travel within the service area

* Valid driver's license and reliable transportation

* Home Health or Hospice experience preferred

* Home Care Home Base experience preferred
