Job Summary

Select Home Health is seeking a skilled and compassionate Weekend Admission Registered Nurse to complete home health admissions and provide exceptional patient care in the home setting. This role is responsible for conducting comprehensive assessments, developing individualized care plans, and ensuring patients receive high-quality, personalized care while promoting independence, safety, and overall well-being.

Duties/Responsibilities:

• Complete home health admissions, comprehensive patient assessments, and ongoing evaluations.
• Develop and implement physician-ordered plans of care while accurately completing all required documentation, including OASIS assessments.
• Provide skilled in-home nursing care to multiple patients throughout the day.
• Educate patients and caregivers on medications, disease processes, treatment plans, and in-home care needs.
• Coordinate patient services and supervise LPNs, HHAs, and other support staff as appropriate.
• Communicate changes in patient condition to physicians and interdisciplinary care team members.
• Ensure compliance with federal, state, and agency regulations, policies, and standards of care.
• Collaborate with the interdisciplinary team to promote positive patient outcomes and continuity of care.

Physical Requirements

• Ability to sit, stand, and drive for extended periods while providing in-home care.
• Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.
• Regular use of a tablet for documentation, communication, and training.

• Graduate of an accredited school of professional nursing (required).
• Current Registered Nurse license for the practicing state (required).
• Minimum of 1 year nursing experience in a hospital, facility, or home health setting; home health experience (preferred).
• Valid driver's license, reliable auto insurance, and personal transportation (required).
• Current up to date TB test.
