What You'll Do

Clinical Care & Assessment

* Perform nursing assessments in community and clinic-based settings

* Monitor physical and behavioral health needs of clients

* Collect medical histories, complete screenings, and record vital signs

Care Coordination & Support

* Collaborate with ACT team members to develop and implement care plans

* Coordinate medical appointments and follow-up services

* Ensure continuity of care across multiple providers

Medication & Treatment Management

* Administer medications and therapeutic injections as needed

* Monitor medication compliance and side effects

* Coordinate with pharmacies and providers for medication access

Education & Advocacy

* Educate clients on medications, chronic conditions, and wellness

* Support disease prevention and harm reduction strategies

* Advocate for client needs within healthcare and community systems

Documentation & Compliance

* Maintain accurate and timely documentation in the EHR

* Ensure compliance with regulatory and organizational standards

Education and/or Experience Qualifications

* Current RN or LPN license in Indiana (or ability to obtain)

* Willingness to obtain licensure in additional states as needed

* Experience in behavioral health, community health, or primary care preferred

Required License/Certification

* Active RN or LPN license

* BCLS certification required

* ACLS preferred

Additional Qualifications

* Strong communication and interpersonal skills

* Ability to work independently in community settings

* Comfort working with individuals experiencing mental health and substance use challenges

* Proficiency with electronic health records

* Ability to manage multiple priorities in a fast-paced environment

* Valid driver's license and ability to travel locally
