Registered Nurse – Home Health
Shift: 8 hours

Requirements:
- Current state license as a Registered Nurse
- OASIS Experience
- Homecare Homebase Software Experience
- CHPN certification (Preferred)
- Proof of current CPR certification
- Valid driver's license
- Reliable transportation and insurance
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years home health experience
Roles & Responsibilities:
- Assess patients to identify their physical, psychosocial, and environmental needs.
- Develop and document the Plan of Care (POC) to ensure continuity of care.
- Educate patients and families regarding care, medications, and home safety.
- Supervise and provide clinical direction to support staff ensuring quality service delivery.
- Monitor patient pain levels and utilize appropriate pain management techniques.
- Maintain communication with interdisciplinary team members, patients, and families.
