Position Overview
We are seeking a compassionate and experienced Registered Nurse (RN) to provide skilled home health nursing services to patients in their homes. The ideal candidate will deliver high-quality patient care, develop individualized care plans, collaborate with interdisciplinary teams, and ensure compliance with Medicare regulations, state requirements, and agency policies.
Key Responsibilities- Perform comprehensive in-home nursing assessments.
- Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Supervisory, and Discharge visits.
- Develop and update individualized patient care plans.
- Provide skilled nursing care, including medication management, wound care, catheter and ostomy care, injections, IV therapy (if applicable), and chronic disease management.
- Educate patients and caregivers on treatments, medications, and disease management.
- Collaborate with physicians, therapists, case managers, and other healthcare professionals.
- Supervise Home Health Aides (HHAs) and Licensed Practical Nurses (LPNs) as needed.
- Maintain accurate electronic documentation.
- Ensure compliance with HIPAA, Medicare guidelines, and state home health regulations.
- Participate in quality improvement initiatives and team meetings.
- Active Indiana Registered Nurse (RN) license in good standing.
- Minimum of 2 years of nursing experience.
- Current CPR/BLS certification.
- Valid driver's license with reliable transportation and proof of auto insurance.
- Strong communication, organizational, and time management skills.
- Ability to work independently in a field-based environment.
- Home Health nursing experience.
- OASIS documentation experience.
- Medicare Home Health experience.
- Experience with Electronic Medical Records (EMR).
- Leadership or supervisory experience.
- Bilingual candidates are encouraged to apply.

- Day Shift
- Weekends as needed
