Position Overview

We are seeking a compassionate and experienced Registered Nurse (RN) to provide skilled home health nursing services to patients in their homes. The ideal candidate will deliver high-quality patient care, develop individualized care plans, collaborate with interdisciplinary teams, and ensure compliance with Medicare regulations, state requirements, and agency policies.

Key Responsibilities
  • Perform comprehensive in-home nursing assessments.
  • Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Supervisory, and Discharge visits.
  • Develop and update individualized patient care plans.
  • Provide skilled nursing care, including medication management, wound care, catheter and ostomy care, injections, IV therapy (if applicable), and chronic disease management.
  • Educate patients and caregivers on treatments, medications, and disease management.
  • Collaborate with physicians, therapists, case managers, and other healthcare professionals.
  • Supervise Home Health Aides (HHAs) and Licensed Practical Nurses (LPNs) as needed.
  • Maintain accurate electronic documentation.
  • Ensure compliance with HIPAA, Medicare guidelines, and state home health regulations.
  • Participate in quality improvement initiatives and team meetings.
Required Qualifications
  • Active Indiana Registered Nurse (RN) license in good standing.
  • Minimum of 2 years of nursing experience.
  • Current CPR/BLS certification.
  • Valid driver's license with reliable transportation and proof of auto insurance.
  • Strong communication, organizational, and time management skills.
  • Ability to work independently in a field-based environment.
Preferred Qualifications
  • Home Health nursing experience.
  • OASIS documentation experience.
  • Medicare Home Health experience.
  • Experience with Electronic Medical Records (EMR).
  • Leadership or supervisory experience.
  • Bilingual candidates are encouraged to apply.

  • Monday-Friday
  • Day Shift
  • Weekends as needed
