The Admissions/Medical Records Nurse (LPN, RN) at Trilogy Health Services oversees admissions intake and medical records within the Nursing Department. You’ll assess new residents, review baseline care plans with nursing leadership, and ensure accurate documentation in Matrix Care.
You’ll work a 12PM–8PM shift at our Indianapolis campus, contributing to high-quality patient care and on-call coverage as needed.
