As a PRN Charge Nurse (RN), you will:
* Manage and coordinate individualized care plans for LTAC patients.
* Delegation to other members of the healthcare team.
* Assess patient conditions and monitor treatment progress.
* Collaborate with physicians, therapists, and social workers.
* Advocate for patients and facilitate communication among all parties.
* Ensure proper documentation and compliance with standards.
* Coordinate discharge planning and care transitions.
* Educate patients and families on care and health management.
* Optimize use of healthcare resources.
* Participate in quality improvement and staff education efforts.
A few must-haves for Case Manager Nurses:
* Active Registered Nurse (RN) licensed with in the state of Indiana
* 2 years of recent and related LTAC, Acute Care or STAC experience, as required by facility
* At least 1-2 full years of Charge Nurse Experience.
* Must be able to work independently, yet together as part of a team
* Must be able to remain calm under pressure
* Proficient at taking vital signs and administering medication accordingly
* Active BLS, physical, TB test, and other medical compliance as required by facility
