We are seeking a dedicated Nurse Practitioner or Physician Assistant to join our Adult Sickle Cell Disease Program at Indiana University Health Medical Group.

Position Details:

  • Full-time, Day Shift
  • Monday - Friday, 8:00am-5:00pm
  • 40% Inpatient / 60% Outpatient



Job Responsibilities :
Clinical Care


• Evaluate, diagnose, and manage adult patients with sickle cell disease across inpatient and outpatient settings.

• Provide follow-up care after hospitalization, including transitional care visits and ongoing disease management.

• Manage acute and chronic complications of sickle cell disease, including:

  • Vaso-occlusive pain crises
  • Acute chest syndrome
  • Chronic transfusion management
  • Iron overload
  • Disease-modifying therapies
  • Pregnancy-related sickle cell care

• Participate in inpatient rounding at IU Health hospitals as needed.

• Provide urgent access visits and infusion-based care within the Sickle Cell Center of Excellence.

• Collaborate with community providers regarding specialized sickle cell care management.
Program Growth & Outreach

• Serve as a clinical resource for expansion of sickle cell services into additional IU Health hospitals and outreach locations.

• Assist in developing and supporting outreach clinics throughout the Indianapolis region and surrounding communities.

• Participate in patient education initiatives and community engagement efforts.

• Support transitions of care from pediatric to adult sickle cell services.

• Help develop standardized workflows and best practices throughout the health system.
Interdisciplinary Collaboration

• Work closely with the program's two existing Sickle Cell APPs to provide seamless, coordinated patient care.

• Collaborate with hematologists, infusion nurses, social workers, pharmacists, case managers, and inpatient teams.

• Participate in multidisciplinary case reviews and quality initiatives.

Qualifications:

• Experience in Hematology, Oncology, Internal Medicine, Hospital Medicine, or Complex Chronic Disease Management.

• Experience caring for underserved or medically complex populations.

• Interest in program development, quality improvement, and healthcare innovation.

Successful candidates will demonstrate:

• Genuine interest in sickle cell disease and health equity.

• Comfort managing medically and socially complex patients.

• Strong critical thinking and clinical decision-making skills.

• Flexibility to provide care across multiple sites and settings.

• Interest in program growth, outreach, and innovation.
