Sevita is seeking a QI Field Services Associate to deliver person-centered support to individuals in home and community settings. In this role, you'll follow individualized service plans, assist with daily living skills, and promote independence and community inclusion. You'll document visits, monitor safety and well-being, and promptly report changes in condition. Working in the field, you will coordinate schedules, travel to client homes and community locations, and use electronic systems to record services.

You'll collaborate closely with clinical staff, case managers, and families, and participate in ongoing training. Sevita offers a supportive, mission-driven culture where your work truly improves lives.

Responsibilities

  • Provide in-home and community-based support services to individuals with diverse healthcare and social assistance needs
  • Follow individualized care and support plans, documenting services and changes in client condition
  • Assist with daily living activities, skill-building, and community integration
  • Monitor client safety, well-being, and behavior, reporting concerns to supervisors and clinical staff
  • Coordinate schedules, visits, and transportation in the field while maintaining timely, accurate records
  • Collaborate with nurses, therapists, case managers, and families to support person-centered goals
  • Use electronic health/record systems and mobile tools to track visits and outcomes
  • Maintain compliance with Sevita policies, confidentiality standards, and regulatory requirements
  • Participate in ongoing training, coaching, and professional development
  • Uphold Sevita's values of empathy, respect, and inclusion in all client interactions
