Case Manager III is responsible for guiding veterans toward higher independence. The role involves assessing, planning, implementing, coordinating, monitoring, and evaluating a collaborative case‑management process tailored to meet each veteran’s unique needs.

Responsibilities
  • Assess veterans referred to the program for housing, employment, education, financial, sobriety support, legal, social, and medical/psychiatric needs.
  • Coordinate care with VA personnel and community partners for eligibility documentation and health services.
  • Facilitate client intake, compile files, and ensure all eligibility documentation is complete.
  • Collaborate with clients to complete Individual Action Plans (IAP).
  • Provide guidance to Case Manager I & II staff on IAPs, intervention strategies, progress notes, and discharge planning during weekly meetings.
  • Conduct crisis intervention and support staff with client issues.
  • Maintain confidential client files in compliance with organization standards and HIPAA.
  • Meet with clients as required, document progress in DAP format, and update IAPs monthly.
  • Complete discharge summaries and conduct home visits to deliver case‑management services.
  • Assist in quality management, data collection, and report preparation.
  • Facilitate client life‑skills classes and specialized groups suitable for veteran subpopulations.
  • Conduct random drug and alcohol screenings of clients.
  • Audit documentation, including eligibility, case notes, IAPs, and discharge forms.
  • Support program reporting and supervise student interns (task assignment, review, evaluation).
  • Train new staff and provide ongoing staff development.
  • Respond to urgent client issues outside of business hours as needed.
  • Enter data into the HMIS system and other required databases; document client referrals.
  • Attend scheduled team meetings and trainings; submit time sheets and related documentation.
  • Perform additional duties as required, including vehicle use for client transportation when authorized.
Program‑Specific Duties
  • Transitional Housing Clients – Drive case management toward permanent housing, ensure daily client presence, monitor rent compliance, and locate community housing units for scattered‑site housing.
  • Permanent Housing Clients – Facilitate acquisition and maintenance of permanent housing, locate community units for scattered sites, and ensure children are enrolled in school.
  • Workforce Clients – Assist with assessment, enrollment, and development of employment plans; provide resume and cover‑letter support; facilitate job‑placement and related classes.
  • SSVF Clients – Deliver rapid‑rehousing services, create Housing Stability Plans, manage family needs, and coordinate VA grants for immediate placement.
  • Aftercare Case Management Clients – Conduct home visits, provide educational activities, refer to supportive services, and coordinate case conferencing.
  • Incarcerated or Recently Released Veterans – Conduct assessment and enrollment, develop housing and employment plans, facilitate program classes, and pass background checks for jail/prison clearance.
  • Counseling Services – Facilitate group or individual counseling and maintain required credentials (e.g., ACSW, LCSW, AMFT, LMFT, or equivalent).
Qualifications
  • Masters Degree in Social Work, Psychology, Counseling, or related field.
  • At least two years of experience in social services or related field.
  • Certification in Substance Abuse Counseling preferred; required for counselors.
  • Background clearance required for work with incarcerated veterans.
  • Licensed practical or registered nurse credentials required for nursing services.
  • Experience with homeless and/or veterans preferred.
  • Experience facilitating support groups preferred.
  • Strong oral and written communication skills.
  • Professional appearance and demeanor.
  • Ability to work independently and within a team, take direction, and serve a diverse client base.
  • Computer proficiency in Microsoft Office and internet research.
  • Valid driver’s license preferred; must meet company insurance and training requirements if operating vehicles.
