As a Weekend Hospice Visit Nurse , you'll provide the reassurance, clinical expertise, and compassionate care that helps patients remain comfortable and families feel supported when they need it most.

• Complete routine and PRN hospice visits in patients' homes, assisted living communities, memory care, and skilled nursing facilities.
• Assess changes in condition and provide expert pain and symptom management.
• Deliver wound care, medication education, and comfort-focused nursing interventions.
• Support patients and families through compassionate communication and education.
• Coordinate care with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team.
• Complete timely, accurate EMR documentation .

• Active Minnesota Registered Nurse (RN) license.
• Hospice experience preferred, or experience in home health, oncology, ICU, palliative care, or end-of-life care .
• Strong assessment, communication, and clinical decision-making skills.
• Ability to work independently in a field-based role.
• Reliable transportation and a valid driver's license.
