- Complete routine and PRN hospice visits in patients' homes, assisted living communities, memory care, and skilled nursing facilities.
- Assess changes in condition and provide expert pain and symptom management.
- Deliver wound care, medication education, and comfort-focused nursing interventions.
- Support patients and families through compassionate communication and education.
- Coordinate care with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team.
- Complete timely, accurate EMR documentation .
- Active Minnesota Registered Nurse (RN) license.
- Hospice experience preferred, or experience in home health, oncology, ICU, palliative care, or end-of-life care .
- Strong assessment, communication, and clinical decision-making skills.
- Ability to work independently in a field-based role.
- Reliable transportation and a valid driver's license.
