Compensation$30 – $38 per visit

Responsibilities- Provide patient care according to client’s care plan and needs
- Perform wound care treatments and assist clients with medication management
- Teach the client and/or family members about care and support skills
- Communicate regularly with RN Case Managers, Supervisors, and the Care Team
- Licensed as a Practical Nurse / LPN in the state of Minnesota
- Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License
- Demonstrated ability to work autonomously
- Prior Nursing experience in a TCU, Hospice, or Home Health setting
- Experience with Electronic Medical Records (EMR) software is preferred
