What You'll Do- Provide skilled nursing visits in patients' homes.
- Perform wound care, medication management, and other skilled nursing interventions.
- Educate patients and family members on treatments, medications, and disease management.
- Collaborate closely with RN Case Managers, physicians, therapists, and the interdisciplinary care team.
- Document visits accurately and ensure continuity of care.
- Active Minnesota Licensed Practical Nurse (LPN) license.
- Ability to work independently while providing compassionate, patient-centered care.
- Reliable vehicle, valid driver's license, and current auto insurance.
- Previous experience in skilled nursing, TCU, rehab, hospice, or home health is preferred.
- EMR experience is a plus, but we'll help you learn our systems.
