Aftercare Case Manager

Sign-On Bonus: $1,000–$1,500

Average hourly rate is $26.43 (including wage, incentives, bonuses, overtime, shift differential, etc.)

• A bachelor’s degree in human services.
• A valid driver’s license.
• A willingness to learn—we’ll provide all the training you need!

As an Aftercare Case Manager, you’ll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care.

• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and resolve any concerns.

• Is compassionate and committed to strengthening families.
• Can help families connect with the right community resources to maintain stability.
• Is an advocate for the families and children we serve.
• Is eager to learn and grow in their skills to better support families and children.
