Responsibilities- Apply nursing theory to assessment, diagnosis, treatment, and evaluation of patient responses to health and illness.
- Assess patient health care preferences and expectations.
- Collaboratively coordinate the continuum of care across settings.
- Develop patient care plans based on patient condition, age, psychological, educational, and socioeconomic conditions that prescribe interventions to attain expected outcomes.
- Implement patient care plans according to patient priorities, including interventions identified through nursing diagnoses, and document/report interventions and patient/family responses.
- Collaborate and integrate contributions of patients and families into multidisciplinary patient care plans.
- Develop, coordinate and implement transfer/discharge planning throughout episode of care.
- Implement nursing interventions according to department, division, and/or unit policies, procedures, and protocols to maintain a safe environment for patients, visitors and staff. Follow universal precautions.
Education:
An Associate's Degree in Nursing, a Diploma in Nursing, a Baccalaureate Degree in Nursing, Professional Masters of Nursing and Healthcare Practice (MNHP), MSN/Clinical Nurse Leader or a Master's Degree in Nursing (MSN, MA) is required.
- Excellent written and verbal communication skills.
- Demonstrated excellent interpersonal skills with patients/families, nursing staff, and interdisciplinary team members through written and verbal interactions.
- General computer skills.
- Current license to practice nursing in Iowa, required by date of hire.
- Baccalaureate Degree in Nursing or Masters Degree in Nursing is highly desired.
- If Baccalaureate degree in Nursing is not yet obtained, active enrollment in a BSN program is desirable.
As needed for clinical coverage. Clinic hours:
Monday-Friday 8:00am-5:00pm.
