We’re not just a job, we’re a place to belong. Iowa City Hospice is seeking a caring and skilled Part Time Licensed Practical Nurse (LPN) who is passionate about providing personalized, high-quality hospice care in a respectful and supportive environment.

As a Hospice LPN, you will provide skilled nursing care to patients with life-limiting illnesses in their homes or care facilities. You will play a vital role in supporting comfort, preserving dignity, and enhancing quality of life while guiding and supporting families throughout their journey.

Key Responsibilities
  • Deliver comprehensive, patient-centered care that honors each individual’s goals, comfort, and dignity.
  • Perform thorough clinical assessments and develop individualized care plans that evolve with patient needs.
  • Administer medications and manage complex symptoms to ensure comfort and safety.
  • Serve as a compassionate advocate for patients and families, supporting informed end-of-life decisions.
  • Educate families on disease progression, symptom management, and caregiving techniques.
  • Collaborate closely with physicians and interdisciplinary team members to coordinate holistic care.
  • Provide emotional and psychosocial support to patients and loved ones.
  • Document care accurately in accordance with hospice standards and regulations.
  • Practice with professional autonomy, confidence, and compassion.
  • Contribute to a supportive, collaborative team culture focused on excellence in care.

Requirements / Qualifications
  • A graduate of an approved school of nursing with a current Iowa Licensed Practical Nurse (LPN) Nursing License.
  • Two years of home health or hospice or work related experience preferred. General medical knowledge related to home health and hospice services with end-of-life issues is desirable.
  • Car, driver's license and adequate insurance as outlined in the personnel policies is required.
  • Strong clinical knowledge, critical thinking skills, and evidence-based practice expertise.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to prioritize tasks, delegate responsibilities, and manage multiple priorities effectively.
  • Proficient computer skills, including electronic health records (EHR) and Microsoft Office applications.
  • Advanced Certifications in Hospice and Palliative Nurse certification by the 3rd year of employment.
  • All offers of employment are conditioned upon the result of a criminal background check conducted by the Iowa Division of Criminal Investigation, as required by law and a negative finding on the Office of Inspector General’s Excluded Provider List.
