As a Hospice Nurse Case Manager (RN), you will coordinate and deliver individualized, patient-centered care in patients' homes and care facilities. You'll build meaningful relationships with patients and families while working both independently and collaboratively with an experienced interdisciplinary team.
In this role, you will help ensure comfort, dignity, and quality of life while guiding patients and their loved ones through the caregiving journey with compassion, confidence, and clinical expertise.
Key Responsibilities include:
- Develop, implement, and oversee individualized plans of care based on each patient's goals, needs, and preferences.
- Deliver comprehensive nursing care in home and facility settings.
- Coordinate medical, emotional, psychosocial, and spiritual support services with the interdisciplinary team.
- Perform ongoing assessments and adjust care plans as patient needs evolve.
- Administer medications and manage complex symptoms to promote comfort and quality of life.
- Educate patients and families regarding disease progression, symptom management, and caregiving strategies.
- Serve as a trusted advocate and resource for patients and families navigating end-of-life decisions.
- Collaborate closely with physicians, advanced practice providers, and interdisciplinary team members.
- Provide emotional support and compassionate guidance throughout the hospice journey.
- Maintain accurate and timely clinical documentation in the electronic medical record.
- Participate in interdisciplinary team meetings and quality improvement initiatives.
- Practice at the top of your license with autonomy, accountability, and confidence.
