PA​/ARNP Hospitalist - Respiratory Specialty & Comprehensive Care Unit

Basic Functions and Responsibilities

Provide patient care in the Pulmonary Respiratory Specialty & Comprehensive Care Unit (a critical care step‑down unit), Pulmonary Procedural Services, and outpatient follow‑up of these patients. Offer overall medical care and management, exercising autonomy in medical decision‑making and services. Function independently to plan, deliver, and evaluate the required health care for the patient, focusing on maximizing resource utilization and controlling costs while providing personalized care.

Coordinate care with nursing, social work, and consulting services, and assist patients in transitioning to outpatient settings by developing relationships with outpatient care providers.

• Provide day‑to‑day medical management and serve as the primary patient manager, including admissions, orders, plans of care, and discharge plans.
• Promote evidence‑based care guidelines for critically ill patients, including sedation, thrombosis prophylaxis, central line management, and ventilator management.
• Perform initial evaluations of newly admitted patients, including history, physical examination, and admission orders under attending physician direction.
• Direct patient evaluations and treatment plans in collaboration with attending physicians from the MICU, RSCCU, and Interventional Bronchoscopy staff.
• Perform or assist with diagnostic and therapeutic procedures, document preparation and response, and participate in daily inpatient rounds.
• Complete written medical record documentation, including progress notes and discharge summaries, and coordinate discharge planning and post‑discharge follow‑up care.
• Mentor and educate fellows, residents, medical students, nurses, physician assistants, and advanced registered nurse practitioners regarding inpatient population management and optimal resource utilization.
• Coordinate and participate in clinical research on topics such as cystic fibrosis, critical care physiology, ventilator management, sepsis, acute lung injury, and length‑of‑stay management.
• Order and interpret diagnostic and laboratory tests, and explain procedures to patients, families, and staff.
• Provide anticipatory guidance and counseling to patients and families, including goals of care.
• Facilitate communication with internal and external health care providers in coordination with the attending physician.
• Initiate and coordinate patient referrals to appropriate specialty and subspecialty physicians or other health care providers as indicated.
• Perform essential weekend and holiday duties on a rotating basis (1:4) with commensurate relief of weekday functions.
• Develop, utilize, and evaluate resources for patient and family education needs.
• Serve as a resource for CCOM and UI Health Care staff and providers, ensuring clinical competency and expertise.
• Maintain effective working relationships with faculty, staff, trainees, and the public.
• Follow policies and procedures in accordance with regulations of the University of Iowa Health Care, Carver College of Medicine, and the Department of Internal Medicine.
• Perform other duties as assigned within the professional scope of practice.

• Physician’s Assistant:
  Must comply with registration and licensure requirements specified by the Iowa Board of Physician Assistant Examiners, including graduation from an accredited PA program and/or NCCPA certification.
• Advanced Registered Nurse Practitioner (ARNP):
  • Master’s degree in Nursing preferred; if Master’s in a related field, a Baccalaureate in Nursing is required.
  • Current certification in an adult specialty area and licensure as an RN and ARNP in Iowa required.
• Excellent written and verbal communication skills required.
• 1–3 years of experience in Acute Care or Intensive Care clinical practice required.
• Demonstrated knowledge of effective strategies for working with diverse faculty, staff, and students.
• Experience providing overall medical care and management in an Acute Care or Intensive Care setting highly desirable.
• Experience exercising autonomy in medical decision‑making highly desirable.
• Experience managing length of stay and resource utilization to reduce costs while providing personalized care highly desirable.
