Clinical Coordinator - Pediatric Specialty Clinic (Neuromuscular)

The University of Iowa healthcare Department of Nursing is seeking a
100% Clinical Coordinator for the Pediatric Specialty Clinic, Neuromuscular team and Limb Girdle Muscular Dystrophy (LGMD) Center of Excellence.

This position will provide nursing leadership and oversight for specialized patient care populations that are served within the Pediatric Specialty Clinic, Stead Family Children’s Hospital, and at Outreach clinic locations. The Clinical Coordinator will use nursing expertise to optimize nursing and healthcare through management of quality, design of informatics, and active participation in the decision-making process. The Clinical Coordinator will facilitate communications between medical staff, management, patients and/or family to provide excellent patient care.

This position will serve as the clinical nurse expert to independently coordinate and facilitate the treatment plan for pediatric and adult neuromuscular patients being seen in the multi-disciplinary neuromuscular clinic that is recognized as an MDA Care Center, a PPMD Certified Duchenne Care Center, and an LGMD Center of Excellence, and those receiving gene therapy or other novel high-cost disease modifying medications.

This position will serve as a liaison role to provide the dissemination of information and education to the clinical care team.

Responsibilities:

• Collaborates with nursing, medical staff and other health professionals in the development and review of standards and in planning, implementing, and evaluating patient care for the pediatric urology patient population regarding the condition and age of the patient
• Independently coordinates and facilitatespatientcare between multiple disciplines within both the confines of UI Health Care andthecommunity (including home health agencies, referring physicians and other community support services)
• Coordinates patient care to better utilize supportive services through patient visit and possible admission, pre-schedules testing, follows up with test results to both patient and physician, obtains medical history, coordinates follow up visit appointment, provides medication, rehab teaching, etc
• Facilitates care of pediatric urology patients requiring admission, coordinating necessary admission procedures, pre and post procedure care
• Operates autonomously in communicating with members of the healthcare team directing patient careand providing continuity of careacross settings throughout the phases of care
• The position involves collaboration in developing communication strategies to promote the TSC, Neurocritical Care, and Neuromuscular programs

Designs and implement patient/family health education activities for the pediatric neurology patient population recognizing individualized need

• Collaborates with nursing, medical staff, and other health professionals in planning, delivering, and evaluating care for specialty patient populations
• Provides direct patient cate/nurse triage responsibilities as needed throughout the PSC
• Performs clinical duties, special projects and other tasks as needed.

Percent of Time:100%
Schedule:Monday-Friday, 8 hour shifts between the hours of 8:00am - 4:30pm. No holidays, no weekends.

Requirements:

• Baccalaureate Degree in Nursing is required
• Current License to practice Nursing
• Minimum 1 year of recent nursing experience

Experience collaborating with an interdisciplinary team

• Demonstrated experience of coordinating care along the healthcare continuum
• Demonstrated experience in patient education
• Excellent written, verbal, and interpersonal communication skills
• Demonstrates leadership and training skills
