Schedule:

Full‑time, Monday‑Friday 8:00am‑5:00pm

• Documenting Patient Encounters (~ 80%)
  • Join the provider in the exam room to observe patient visits
  • Document encounters in a structured note (history of present illness, assessment, plan, physical exam)
  • Assign appropriate CPT and ICD‑10 codes
  • Prepare After Visit Summaries
  • Consult with the provider to ensure accurate and specific documentation
• Clinical Documentation Improvement (~ 10%)
  • Request and review medical records
  • Leverage Oak Street’s population‑health tools to support documentation improvement
  • Prepare for and support Daily Huddles and Documentation Reviews
  • Consult with the provider on documentation improvement opportunities
• Administrative support for provider and care team (~ 10%)
  • Place orders and referrals
  • Address tasks related to clinical documentation
  • Assist the care team with additional responsibilities as assigned

• Knowledge
  • Medical terminology and common medications (required)
  • Pre‑medical degree or prior clinical experience (required)
  • Shadowing or volunteering experience (strongly preferred)
  • Prior scribe or transcription experience (preferred)
• Skills
  • Advanced listening and communication skills (required)
  • Strong computer literacy and learning ability (required)
  • Fluency in Spanish, Polish, Russian, or other community languages (required where indicated)
• Abilities
  • Adapt to new workflows and learn quickly (required)
  • Type 70+ words per minute (strongly preferred)
  • Take direction and work as part of a care‑team (required)
  • Self‑starter within role scope (required)
  • Excellent job attendance, mandatory in‑person work (required)
  • Commit to at least 1 year in the role (mandatory)
  • Work 40–45 hours per week during clinic hours (mandatory)
  • Compliance with hospital and Oak Street Health policies (mandatory)
  • U.S. work authorization (mandatory)
